For everything visible or invisible phenomenon to humans for every effect or action to take place a cause or a root exist.At the physical level all matter exists as effect due to a cause in the particle bond (ion or atom) Even in quantum physics decay and disintegration i caused by loss in the energy level of the orbiting electrons.At the biological level there has to be a cause for life to take effect.Some effect (action) must occur for as cause to give effect in the cyclic principle of effect cause effect principle..

In quantum physics, event can occur without a cause. For example, the decay of a radioactive nucleu doe not follow the rule of cause and effect, and can only be predicted on a statistical basis. Since quantum mechanic i the most fundamental basi of reality, cause and effect falls apart at the atomic scale.

Yes causes always leads to effects, if a cause does not lead to an effect then the cause is not a cause at all but a nothing..

Every cause also had a cause too, except for the very first cause which had to be eternal without a cause to bring it forth..
