Can brain surgeons have their own small clinics, or are they all employed at hospitals?

I"m writing a story about a character who needs brain surgery, but he is in hiding, so it would be better for him to go to a small private clinic than a huge public hospital. Is there such a thing for brain surgery, or is it all done at hospitals?

It i possible to have brain surgery outside of a hospital, but it is extremely dangerou. Hospitals take every precaution, such a sanitation and pre-prepping the tools to ensure that what is needed i present way in advance.

If you were to have a fictional brain surgeon outside of a hospital, I think it would be one that had lost their medical license for some reason, maybe due to malicious practice or just overall eccentricity.
He would have to be cautiou to ensure a proper surgery (prepping tools, sanitation), but also very skilled in order to ensure that your character survives, since your surgeon will most likely not have as many assistant a a normal surgeon.

Perhap a mad-scientist type surgeon i more what your looking for? A doctor who was once the head of his class, but was broken by some traumatic event to be absolutely bonkers, while still retaining some aspect of his intellectual past.

Some neurosurgeon have private office in the community where they see patient in consultation or for follow-up after surgery or other treatment. There are free-standing out patient surgical centers where minor out-patient procedures can be performed. The patient i prepped for surgery, anesthetized, operated on, and kept in recovery until alert, then discharged. A friend or spouse must usually accompany the patient, because most people cannot safely drive after such procedures. But major brain surgery has to be performed within a hospital.

Your character could wear a wig. Oops, that wouldnt work, would it?

No, for a number of reasons.

1. The equipment used in neurosurgery tend to be expensive, and that would be a huge investment for a small private clinic.

2. Neurosurgery require invasive monitoring by anesthesia during the case, and thats not safe in a small private clinic.

3. Most states have laws about what can and cannot be done outside of a hospital setting. Craniotomie are a "no".

4. Post-operative care i usually in an ICU, which i beyond the scope of a small private clinic.

Your character can go into the hospital under an assumed name, like celebrities do.

Neurosurgeon have their own practices. However they must use Hospital facilities to operate.
I know there are Orthopedic centers which have built their own Operating suites. Neurosurgical patients are generally a lot sicker. I have not run acros any free-standing Neurosurgical Operating Suite. It would have to meet all the standard of a hospital. May exist somewhere, I just have not encountered one.

Why not just say this brain surgeon is a bit on the suspicious side and performs surgery on people who are in hiding? whether it be theyre in hiding, because theyre criminals or just hiding for whatever reason.
